Dignity Health-Glendale Memorial Hospital and Health Center encourages local nonprofit organizations to submit letters of intent (LOI) for the annual Dignity Health Community Grants program, which earlier this year awarded $101,000 to four lead organizations supporting their respective accountable care communities.

Dignity Health’s Community Grants Program focuses on awarding funds to provide services to underserved populations including the economically poor, women and children and the mentally or physically disabled.

Once again, Dignity Health-Glendale Memorial Hospital will award grants to Accountable Care Communities involving active collaboration with three or more organizations responding to needs identified in the hospital’s most recent Community Health Needs Assessment. The top nine health concerns identified include obesity/overweight, mental health, diabetes, alcohol and substance abuse, cardiovascular disease, hypertension, cholesterol, disability and oral health. Grants ranging from $10,000 to $100,000 will be awarded to qualified nonprofit organizations. Letters of intent must be submitted no later than July 14.

Grant applicants may download the 2016 LOI form at dignityhealth.org/glendalememorial/ and submit the LOI to Glendale Memorial Hospital. The community grants selection committee will review all LOIs submitted by July 14. In keeping with our strategic priorities, Dignity Health encourages grant applicants with programs that integrate the following principles: access to care with focus on disproportionate unmet health related needs; emphasis on primary prevention – addressing underlying causes of health problems; seamless contribution to continuum of care; building community capacity; demonstrating collaboration.
